Transference-Focused Psychotherapy (TFP)

Transference-Focused Psychotherapy (TFP) is a structured psychodynamic approach in the Kernberg tradition that examines personality organisation live in the transference–countertransference field. The therapist names and works with relationship patterns as they emerge. It does not diagnose; clinical judgment stays with the clinician.

Core concepts include split internal object relations, identity diffusion, primitive defences, and transference as intervention. Interpretations are clear, timely, and relational; intensity may rise in a contained way. Frame, limits, and contract support safety.

The therapist is an active interpreter and frame holder—not promoting boundless regression. Goals target transforming fragmented internalisations toward more integrated object relations. Supervision supports countertransference management.

TFP implies longer-term work; crisis stabilisation and adjunct care are separate decisions. No automated diagnosis, prescribing, or forensic reporting is implied.

Emotional intensity is inherent; acute risk prioritises safety. Not every client is suitable; fit is a clinician judgment.

In PsychSupervisor, TFP frames reports on transference themes, defences, and intervention fit as decision support.

How it appears in the app

In PsychSupervisor you choose Transference-Focused Psychotherapy (TFP) as the theoretical frame when opening a case; the session transcript is read through that lens. The report is organised under five standard headings: Session Overview Summary, Developmental Assessment, Session Evaluation, Theory-Based Evaluation of Therapist Interventions, and Recommended Therapy Plan. After speaker roles are assigned, text is structured in the selected approach; evidence-mode sections stay tied to the transcript, while narrative-mode sections may summarise flow. Outputs support supervision and record-keeping as clinical decision support; they do not assign diagnoses, prescribe medication, or replace emergency care. The clinician verifies the transcript and retains final clinical judgment.
