Sample report
A shortened example from a fictional session, so you can see what a report looks like.
This page is fictional. The session, people and quotes were written for illustration and do not belong to any real client or session. The length and detail of a real report depend on the session and the chosen theory.
Theory: Cognitive Behavioural Therapy (CBT) · Individual session · Client: D. (pseudonym) · Session 4
Session Overview Summary
The session focused on the client's anxiety before presentations at work. D. described how, before a meeting last week, the thought “Everyone will see how inadequate I am” came up and how they handed the presentation to a colleague [06:12]. The therapist explored this example with the client through the cognitive model; in the second half they discussed how avoidance brings short-term relief and keeps anxiety going in the long term [21:40]. The emotional tone was tense at first and became more curious and collaborative towards the end.
Developmental Assessment
The transcript contains little developmental history. D. briefly mentioned being mocked while speaking in front of the class at school [14:05]. A possible link between this memory and the current “inadequacy” belief was noted as a hypothesis to explore later, not as a conclusion. Family history was not discussed in this session.
Session Evaluation
The agenda was set together at the start and last week's homework was reviewed [02:30]. Work on the automatic thought record was consistent with the session goal. The client struggled with the evidence-gathering step (“But I really feel that way” [18:22]); this moment could be seen as an opportunity to work with emotional reasoning. The transcript contains no risk statements.
Theory-Based Evaluation of Therapist Interventions
Socratic questioning [17:10] and psychoeducation about the avoidance cycle [22:05] fit the CBT frame. The therapist's summaries are frequent and clear. The move to a behavioural experiment was squeezed into the last minutes [44:30]; allowing more time to plan the experiment could be considered.
Recommended Therapy Plan
Possible focuses for the next session: building graded exposure steps for pre-presentation anxiety together; continuing evidence and counter-evidence work on the “inadequacy” belief; when the client is ready, exploring the school memory in terms of how the belief developed. A topic for supervision: the timing of the behavioural experiment within the session.
Time stamps in square brackets point to the matching line in the transcript; in the app, a quote opens the transcript at that line.
